Our Services
CDH provides a variety of Group Supported Employment options integrated with Day Support Options to create a unique and individualized experience allowing individuals to thrive and unlock their full potential.
Our program also offers Individualized Home Support creating a person-centered plan to support individuals in achieving their goals and living as independently as possible. In-home and community supports include assistance with daily living, social and safety skills, community integration, relationship building, nutrition, exercise, money management, self-advocacy, and pursuing community recreational activities.

At CDH, we embrace a supportive and compassionate philosophy where everyone can thrive. Our interactive and engaging program fosters collaboration, creativity, and connection, ensuring all participants feel valued, heard, and motivated to contribute. Through welcoming environments that promote respect and belonging, we empower individuals to succeed while building meaningful relationships. In addition, we provide opportunities to learn practical job skills, preparing participants for greater independence and success in the workplace.
Our life skills curriculum utilizes multimodal learning techniques and themes to promote independence, build confidence, and foster job skills. Curriculum topics range from essential skills such as hygiene, personal care routines, building friendships, customer services to advanced skills including financial literacy, problem-solving, planning , and organization.
CDH incorporates movement, meditation, and mindfulness activities to improve self-awareness, emotional regulation, focus, and attention, while reducing stress and anxiety to build a supportive environment that fosters personal growth and resilience.
CDH emphasizes the importance of healthy lifestyles by supporting emotional and mental well-being, nurturing friendships, encouraging physical activity, boosting confidence, and promoting nutritious choices.
Variety of Work Opportunities
Having multiple work opportunities empowers participants with choice, promotes greater engagement, and offers flexibility—while also helping them develop a broad range of job skills for future success. This supports a more personalized and meaningful work experience allowing individuals to choose roles that align with their interests and strengths.

At the restaurant, there are many roles, from food prep to working the register. Our individuals prepare breakfast and lunch service and get creative with specials and desserts while building cooking skills for both work and home. Additional kitchen tasks include cooking, taking orders, running the register, inventory, and cleaning to keep the kitchen running smoothly.

At the Blue Goose Storefront, we offer a wide selection of handmade gifts, from seasonal décor to handcrafted jewelry. Individuals design and create products, manage displays, take product photos, assist customers, and handle checkout, gaining valuable retail experience.

In the print shop, we design and create custom items such as tumblers, shirts, and wind spinners. Using tools including the sublimation printer and Cricut, individuals bring their unique designs to life.

Our seasonal site is a hands-on space where we handle everything from planting to harvesting. Exploring a variety of gardening tools and tasks to produce lots of vegetables. This year, we harvested acorn squash and potatoes for shared meals, while fresh tomatoes and jalapeños were made into pico de gallo at the restaurant for true farm-to-table freshness!
Exploring Interests and Team Building
Our approach to team building is grounded in the values of respect and wellness, with connection at the heart of everything we do. Daily clubs, weekly shared meals, and group celebrations offer meaningful opportunities for collaboration and relationship-building. The Wellness Studio provides a dedicated space for adaptive fitness, self-care, and personal growth to support both physical and emotional well-being. Together, these experiences foster a strong sense of community, empowering participants to explore their interests, develop healthy relationships, and contribute to a respectful environment.

At CDH, we enjoy daily club time. Every afternoon individuals choose from a variety of clubs to explore their interests and try new activities. Some clubs involve community outings, such as library or walking club. We strive to better the environment with recycling club and explore a variety of interests with cooking club, crafting, and more.

Our wellness studio is a safe and fun space designed to support physical, emotional, and mental well-being. We offer adaptive fitness, which is individualized to all abilities. Through personalized programs, we promote fitness, relaxation, social connection, and empowerment, fostering a sense of belonging and confidence for every individual.

As part of team building, we believe sharing quality time and family style meals adds a sense of community. We enjoy weekly meals such as coffee and a bite on Monday mornings, lunches on Fridays. Participants choose meals, make them, and serve them to their peers.

Celebrating birthdays, holidays, and hosting luncheons together fosters a strong sense of community and belonging. We also hold festive holiday parties and our popular Halloween costume dance, adding fun and excitement to the calendar. These shared moments create meaningful connections, encourage interaction, and build a supportive environment where everyone feels valued.
Our Story
At the Corner of Determination and Hope, LLC is a provider of vocational training and day services for individuals with disabilities located in Plainfield, CT. We offer a variety of job opportunities in a restaurant setting (Route 12 Taco) and retail storefront (The Blue Goose), both located at our main site. We are dedicated to providing meaningful employment opportunities in a supportive environment which fosters independence and personal development.
We started this journey in 2015 with a small, roadside taco shop on Route 12. At that time, my youngest child was preparing to transition out of high school, and for individuals with disabilities, finding a safe and supportive place of employment can be a challenge as not every work environment is able to meet individual needs. The restaurant became the place where she could work and have daily opportunities to learn new job skills and develop friendships. In 2018, our family made the decision to expand this vision and open a full program that provides vocational and day support opportunities for many individuals with disabilities in our community.
The program consists of vocational opportunities, wrap-around support, and life skills training. Our company creates individualized programming for each person that we serve. Additionally, we provide education and training for activities of daily living, social communication, creative expression groups, wellness activities, and community based recreational opportunities.
